What data we collect and how we store it:
We need to collect personal information about your health to provide you with the best possible treatment. We record and use the following categories of personal data: Name; address; date of birth; email address and telephone number; health information including past medical history and any known illnesses. Under the Chiropractors Act 1994, we are required to collect personal and sensitive data as an essential part of our care. You can of course refuse to provide the information, but we would be unable to treat you If you are to do so.
We will never share your data with anyone who does not need access without your written consent. Your records are stored in secure servers in a private server bank. Data is encrypted when sent between our devise and servers, just as the way your banking information would be. Only clinic personal will have access to this data.
Retaining your personal data:
We are required by law to retain your records for 8 years after your most recent appointment (or age 25, if this is longer). After this period you can ask us to delete your records if you wish.
Your rights:
You have the right to request a copy of your data held by us at any time. If you believe any of the personal information we hold about you is inaccurate or incomplete, please email the Data Controller directly and any necessary corrections to your data will be made. When making a request please provide the following information: your name, telephone number, home address, email address and details of the information you require. If you would like to request the erasure of your data, (subject to our legal 8 year retention period), please contact the data controller on colette@elychiropractic.co.uk
Data breaches:
Should your personal data that we control be lost, stolen, or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay. We will explain to you the nature of the breach and the steps we are taking to deal with it.
Complaints:
If you feel we have mishandled your personal data, please contact our Data Controller and we will discuss and action any queries you may have. If you are not satisfied with our response, you have the right to contact the Information Commissioners Office (ICO) via their website: www.ico.org.uk
